Nominations may be made by e-mail or by letter

When nominating someone it is important that you should give sufficient detail to allow the judges to make a decision and, as a guide, we suggest that you should provide information on your nomination as follows:

• Who are you nominating: Name/organisation.
• Where the nominee gives support and the length of time your nominee has been doing this.
• Explain why you are nominating this person/group. Please give as much detail as possible as the more information you give the better for the judges to make their decision.
• Your name, telephone number and e-mail address (if available).

   a Word File or as a PDF File.

Nominations open until 5pm Friday 20th November and can be made online by……
